Community Habilitation
Community Habilitation provides at home and community services to developmentally disabled children and adults. This one to one service provides individualized support to those diagnosed with a developmental disability. Each individual utilizes the program slightly differently. Some utilize the service for grooming or behavioral needs, while others use the resource for recreational and community integration activities. Our staff will work closely with each family to provide the desired service they request. This service is provided anywhere from 12 to 30 hours a week.
Respite Services
Respite services provide temporary relief from the demands of care giving, which helps reduce overall family stress. This often enables families to better meet the needs of their loved one with a developmental disability. Respite can be provided in the home or out of the home, during the day, evenings or overnight.
It is an “indirect” service that provides relief to individuals responsible for the primary care and support of an individual with a developmental disability. When the person providing the care has to deal with an illness, emergency, and care giver or staff vacation, respite services can ensure that their loved one’s needs are met.
Day Habilitation / Cardio Club
Day Habilitation provides Functional, Social, Personal Development and Community Awareness skills to developmentally disabled adults ages 18 and up. Our participants experience a wide range of recreational/educational field trips, computer, art, and volunteer opportunities in and around the Mott Haven community. We also provide occupational, speech and behavioral therapies. Our program is dedicated to providing quality experiences for those who are often overlooked in society. We support and encourage each participant to strive to be ALL they can be-‐ regardless of the challenges they may face.
The Cardio Club otherwise known as the “Cardio Club” officially opened in June 2008. It is health and fitness club for people with developmental disabilities. The club provides an environment where the physical fitness and healthy life styles needs of the participants are addressed. It also provides vocational and day habilitation experience for the Peer Trainers. Those deemed Peer Trainers have the opportunity to learn how to run the day-to-day operations of a business.
Employment Services
Individualized relationship between individuals seeking employment and employers. By individualizing the relationship, work experiences are tailored to match the interests, strengths and goals of each individuals. Necessary needs for support are assessed making it possible for each candidate to have access to employment services, regardless of their disability. Employment Services offered include Supported Employment (SEMP, Employment Training Program (ETP), and Community Pre-Voc.
RESIDENTIAL SERVICES
In conjunction with families and advocates, the Residential services department at Creative LifeStyles, is dedicated to delivering quality supports and services to adult individuals with developmental disabilities. In our Individualized Residential Alternatives (IRA), we provide 24-hour care designed to support, develop and enrich the lives of the people we service. We currently provide residential care to 41 adults amongst 9 residences in the Bronx, NY. We encourage and foster independence through skill attainment in all areas of activities of daily living. For over 19 years, it is has been Creative LifeStyle’s goal to provide individuals with the best and most creative lives imaginable as they reach their valued outcomes in life.
